I would say I'm sorry for you having to go through this but after having a friend (girl) go through something very similar, I learned that YOU have to fix YOUR situation. You've obviously tried talking to him and it won't work. Have you ever looked him in the eyes and told him you're leaving until he gets help? If so, it obviously didn't work and you didn't do as you threatened. Guys sometimes take women for granted after a while and become "deaf" to their concerns.
